Once downloaded fill in the appropriate blanks, save it on your computer and print it out.  Once printed drop it off and return it to the Clubhouse Office mail slot.

You can either print out a pink slip form and hand write it, or fill it in on your computer and print it out.  DO NOT EMAIL IT ANYWHERE!!  Drop it off in the Clubhouse Office mail slot.  Please remember if you email it, it will not be looked at.
